Health insurance subsidy in Ticino: who qualifies for Ripam and how to apply

Applications must be submitted by 31 December of the year before the one the subsidy is claimed for. Some groups receive the reduction without doing anything.

Ripam is the reduction of health insurance premiums: a public contribution for people who struggle to pay their health insurance and live in Ticino. The decision rests with the Social Insurance Institute, which assesses the applicant's economic and family situation.

The subsidy does not reach the beneficiary's bank account. The canton pays it directly to the health insurer, which reduces the premium accordingly.

Who does not need to apply

Some groups receive Ripam automatically and need not submit anything. They are people receiving supplementary benefits to the old-age or disability insurance, people receiving supplementary child allowances (AFI) or early childhood allowances (API), people on social assistance and people receiving extraordinary unemployment benefits. In these cases the relevant services handle it directly.

Everyone else has to apply. Before filling in the form it is worth using the online calculator provided by the Institute, which shows whether an entitlement exists without opening a case.

The deadline that matters

The deadline is 31 December of the year preceding the one for which the subsidy is claimed. Anyone wanting Ripam for 2027 therefore has to act by the end of 2026.

The deadline matters because a late application does not recover the months lost. If the request is submitted during the year concerned, entitlement begins only from the month after the application. Applying in summer rather than autumn changes nothing; applying late does.

The form is obtained through the Social Insurance Institute's portal, which publishes updated instructions each year along with the paperwork.

The issue weighs more heavily in Ticino than elsewhere. The canton still has the highest spending per insured person in Switzerland, and the share of household income absorbed by premiums is among the highest in the country. Ripam is the instrument through which the system partly corrects this imbalance, but it only works if those entitled to it apply in time.
